RESUMO
Introducción: El tratamiento y cicatrización de las heridas en personas enfermas de cánceres complejo, llevándonos potencialmente al fracaso, tanto por la naturaleza y efectos de la enfermedad oncológica, como por su proceso y tratamiento de quimioterapia y/o radioterapia. Por ello, nuestra intención de evaluar la idoneidad de nuevos apósitos, como el hidrocelular trilaminar con adhesivo de gel suave, en el cuidado de la úlcera tumoral mamaria. Caso clínico: Estudio de caso descriptivo sobre el uso del apósito hidrocelular trilaminar, con adhesivo de gel suave, en una úlcera tumoral mamaria secundaria a carcinoma ductal infiltrado a piel, con mama izquierda pétrea. Se valoró: cantidad de exudado, mal olor, características de la superficie de contacto, sangrado, así como bienestar social y psicológico. Por síntomas de disconfort, decidimos cambiar el tipo de cura tradicional con apósito de gasa por el cuidado con el apósito hidrocelular. El seguimiento fue de 6 meses, con 2 a 3 visitas semanales, donde fomentábamos el autocuidado y uso correcto del apósito. En las primeras curas redujimos el mal olor y se mejoró la comodidad del apósito al disminuir el grosor, pues el hidrocelular daba mayor movilidad a la mujer, ajustándose mejor, y corrigiendo así también la estética. La zona de contacto de la úlcera con el apósito mantuvo la humedad idónea, controlándose el exudado, permitiendo que la mujer volviera a salir a la calle sin miedo a manchar su ropa. El suave adhesivo del apósito permitió la permanencia en contacto con la herida, y facilitó la retirada no traumática del mismo, sin mostrar dolor ni sangrado. Se logró espaciar las curas c/48 h, y 72 h al final del periodo de seguimiento. Conclusiones: El uso del apósito hidrocelular trilaminar, con adhesivo de gel suave, resultó idóneo para el cuidado de la úlcera tumoral, mejorando los síntomas y la satisfacción de la persona por los cuidados recibidos (AU)
Introduction: The treatment and healing of wounds in cancer patients complex, potentially resulting in failure, as much due to the nature and effects of the oncological disease, as to its process and the chemotherapy and / or radiotherapy treatment. For this reason we attempt to evaluate the suitability of new dressings, such as the trilaminar foam dressing with soft gel adhesive in malignant mammary wounds. Clinical case: Descriptive case study on the use of trilaminar foam dressing, with soft gel adhesive, in the care of a breast tumour ulcer secondary to infiltrating ductal carcinoma of skin, with a calcified left breast. We evaluated: the amount of exudate, odour, surface characteristics of contact bleeding, and social and psychological welfare. Due to symptoms of discomfort it was decided to change the type of traditional care (gauzed ressing), into the care with the trilaminar foam dressing. The follow-up was 6 months, with 2to 3 weekly visits, during which we promoted self-care and correct use of the dressing. The first dressings reduced the bad odour and comfort was improved by decreasing the thickness dressing, as the hydrocellular dressing gave the women greater mobility, better adjustment and correction, and aesthetic effect. The contact zone of the wound with the dressing was kept at the ideal humidity, controlling the exudate, enabling the women to go out without fear of soiling their clothes. The soft adhesive dressing remained in contact with the wound, and was painless to remove, showing no pain or bleeding. The dressings were changed every 48 h, and72 h at the end of the period of follow-up. Conclusions: Use of trilaminar foam dressing, with soft gel adhesive, was suitable for malignant fungating wounds, improving the symptoms and the patient satisfaction with the care (AU)
